Default Bladder cancer -- why can't someone swallow?

My grandfather has bladder cancer, and he is now having trouble swallowing. He won't go to the doctor (he is not receiving medical treatment), so I can't ask the doctor. My best guess would be that his lymph nodes are swollen and are acting almost like an imposing structure on his trachea... but that's just a guess. Does anyone know why he might be having difficulty swallowing?
