Default 81yo female with newly diagnosed thyroid medullary cancer. Should I opt for surgery
or radiotherapy?

Depends on lots of factors, including cancer grade and stage, patient's overall health, willingness of surgeon/anesthesiologist to perform surgery, etc. If you're the patient then this decision is totally between you and your doctor.
